The extension INDICATES what the file content is, it doesn't dictate it. Changing a jpg file to doc just makes it a useless file. Programs will open it as a doc file, find that it's not and report it as a corrupt file - which it is for a doc file. It has to be in doc format internally to be a doc file.
An OCR program will scan a picture, attempt to recognize text and output a text file (in whatever format you select). But even with the best OCR program (read: $$$) you'll have to make manual corrections. Even if the jpg is high contrast, no noise, O and 0 (and ) and () get mixed up, so do 1 and l (and !). The worse the program, the more you'll have to correct.
