Let me change this a bit. If its at the point where the phone is asking for the PUK once you put it in it will ask you to chnge the pin. If you've already entered it wrong too many times then the sim is fried and you'll need a new one, in which case you won't need to know the pin anyways.
All you need is a new sim card, the phone is fine.
